What is GP2GP?
GP2GP is a secure digital system that transfers your complete electronic health record when you change GP practices. This ensures your new doctor has your full medical history available at your first appointment.

How It Works
1. When you register at a new practice, GP2GP automatically requests your records from your old GP
2. Both practices must use GP2GP for electronic transfer
3. Your paper records will follow separately (currently still required)

What Information Is Transferred?
Your complete digital health record including:
- Medications and prescriptions
- Allergies and adverse reactions
- Vaccination history
- Test results and diagnoses
- Specialist letters and medical history
