Your Summary Care Record (SCR): What You Need to Know

What's in Your SCR?
Your Summary Care Record contains essential medical information to help healthcare professionals provide you with safe, effective care, including:
- Current medications
- Known allergies
- Previous adverse reactions to medicines
- Basic details (name, address, date of birth, NHS Number)

You and your GP can also add additional health information if agreed.

Why It Matters
Your SCR helps authorized clinicians across England make better decisions, especially in emergencies or when your GP practice is closed. This system has helped prevent medical errors and ensures continuity of care.

Security & Access
Healthcare staff can only view your SCR if:
✔ They are directly involved in your care
✔ They have secure NHS Smartcard access
✔ They ask your permission first (unless in an emergency)
All access is tracked for your safety.

Your Choices
Opt in (default): No action needed – your SCR is automatically created
🚫 Opt out: Complete and return an [opt-out form (PDF)] to your GP practice
*You can change your decision anytime by informing your practice.*

For Parents/Guardians
If your child is under 16:
- Discuss the SCR with them
- Consider consulting their GP before opting out
- Understand the implications of not having an SCR
